The Care Co-ordinator specialises in care needs. We can offer help with elderly and frail patients. We are able to advise and signpost patients to different organisations to help them. This includes: Physio, Social Care, Carers needs, Safeguarding, OT assessments and supporting Carers with information to help care for their family. We help to network with agencies and social care professionals that will assist patients to remain safe at home and avoid hospital admission.
We can also help support social isolation & loneliness and a whole other host of issues that you may not need to see your GP for, please contact the surgery and ask to speak to one of our Care Co-ordinators.
